DOT & FMCSA Monthly Safety Inspection

Comprehensive monthly safety inspection checklist for DOT and FMCSA compliance covering all fleet vehicles and operational safety requirements.

REGULATORY COMPLIANCE NOTICE

This inspection must be conducted monthly in accordance with 49 CFR Part 396.17. All defects must be documented and corrected before vehicle operation. Failure to comply may result in significant fines and out-of-service orders.

Pre-Inspection Documentation

  • Verify vehicle registration and insurance are current.
  • Check driver's license and medical certificate validity.
  • Review previous inspection reports and repair records.
  • Ensure all required permits and placards are present.
  • Document vehicle mileage and operating hours.
  • Verify driver vehicle inspection report (DVIR) completion.

Engine & Mechanical Systems

  • Check engine oil level, condition, and for leaks.
  • Inspect coolant level and system for leaks or damage.
  • Verify proper belt tension and condition (no fraying or cracking).
  • Test engine idle and check for unusual noises or vibrations.
  • Inspect exhaust system for leaks, damage, or loose components.
  • Check transmission fluid level and condition.
  • Verify proper operation of engine cooling fan.
  • Inspect air filter and replace if necessary.

Braking System

  • Test service brake pedal travel and firmness.
  • Check parking brake operation and adjustment.
  • Inspect brake fluid level and condition.
  • Examine brake lines and hoses for leaks or damage.
  • Test air brake system pressure build-up (if equipped).
  • Check brake chamber push rod travel and adjustment.
  • Verify proper operation of brake warning lights.
  • Inspect brake pads/shoes for wear and proper thickness.

Steering & Suspension

  • Check steering wheel play (maximum 10 degrees for power steering).
  • Inspect steering linkage for excessive wear or damage.
  • Verify power steering fluid level and condition.
  • Examine suspension components for cracks or damage.
  • Check shock absorbers for leaks and proper operation.
  • Inspect leaf springs and U-bolts for integrity.
  • Verify proper wheel alignment visually.
  • Test steering return to center after turning.

Lighting & Electrical Systems

  • Test all exterior lights (headlights, taillights, brake lights).
  • Verify proper operation of turn signals and hazard lights.
  • Check clearance and identification lights for functionality.
  • Inspect backup lights and reverse alarm operation.
  • Test interior lighting and dashboard warning lights.
  • Verify battery terminals are clean and secure.
  • Check alternator charging system operation.
  • Ensure all reflectors are clean and properly mounted.

Tires & Wheels

  • Check tire pressure on all wheels (including spare).
  • Inspect tread depth (minimum 4/32" front, 2/32" rear).
  • Examine tires for cuts, bulges, or irregular wear patterns.
  • Verify all lug nuts are tight and properly torqued.
  • Check wheel rims for cracks, bends, or damage.
  • Inspect valve stems and caps for damage.
  • Ensure tires are properly matched and rated for load.
  • Check for foreign objects embedded in tires.

Body & Structure

  • Inspect body panels for damage, rust, or structural issues.
  • Check all doors for proper operation and security.
  • Verify emergency exits function properly and are unobstructed.
  • Inspect windows for cracks or damage affecting visibility.
  • Check mirrors for proper adjustment and cleanliness.
  • Verify bumpers are securely attached and undamaged.
  • Inspect frame and cross-members for cracks or damage.
  • Check fuel tank mounting and cap security.

Safety Equipment

  • Verify fire extinguisher is present, charged, and accessible.
  • Check first aid kit contents and expiration dates.
  • Inspect emergency reflectors (3 required) and storage.
  • Test horn operation and volume compliance.
  • Verify seat belts function properly and are not damaged.
  • Check wheelchair lift operation (if equipped).
  • Inspect emergency communication equipment.
  • Ensure all required safety placards are posted and legible.

Driver Qualifications & Records

  • Verify driver has valid CDL with appropriate endorsements.
  • Check medical examiner's certificate validity and compliance.
  • Review driver's record of duty status (logbook/ELD).
  • Confirm driver has completed required training and certifications.
  • Verify driver is not disqualified or on restricted duty.
  • Check drug and alcohol testing compliance records.
  • Ensure driver vehicle inspection reports are current.
  • Review any recent violations or incidents.

Post-Inspection Requirements

  • Complete and sign monthly inspection report form.
  • Document all defects found and repairs needed.
  • Place vehicle out of service if critical defects are found.
  • Schedule necessary repairs with qualified mechanics.
  • Update vehicle maintenance records and database.
  • File inspection report in vehicle's permanent record.
  • Notify drivers of any restrictions or out-of-service status.
  • Schedule follow-up inspection after repairs are completed.
